Post-Permanency Support Services

We believe every child deserves the love, safety, and unconditional commitment of a family and the opportunity to develop as a healthy and resilient young adult. Our Pathways to Permanency team is passionate about supporting adoptive families, guardianship families,  resource families, birth parents, and children in guardianship or who have been adopted through the child welfare system.

Permanency is a lifelong journey, and we are dedicated to building a welcoming community where families can thrive. We aim to empower and uplift families with accessible, tailored resources designed to meet your unique needs.

Post-Permanency Support Services Include:

  • Referrals and connections to therapeutic, educational, and community-based resources.
  • Family Care Coordination and Case Management to help families navigate services, overcome barriers, and strengthen long-term permanency.
  • Educational workshops and trainings (in-person and virtual)
  • Monthly support groups.
  • Crisis support and safety planning.
  • Support understanding the Adoption Assistance Program (AAP) and completing county and state forms.
  • Individualized guidance and support to help families navigate adoption, guardianship, changing family dynamics, identity development, and other permanency-related challenges.

Who Can Receive Services?

Your family may be eligible if you are:

  • A pre-adoptive or pre-guardianship family within 3 months of finalization.
  • An adoptive or guardianship family with a child or youth under age 26 whose permanency was achieved through the child welfare system.
  • A former Wendy’s Wonderful Kids family (from any state).
Service Area:
  • Families living in Sacramento County and surrounding areas
